If you have copied and pasted over text into a contacts History and see that on the Dashboard the information contains code such as that shown above in red then this issue is to do with the formatting of text when it is copied over into a History item.
To work around this just re-open the history and re-save it by clicking on 'Update' as shown below which will fix this issue.
You will see that the text on the Dashboard and inside the History for your contact is now shown:
